Partners in Senior Care, Inc. introduces the Navigator Program for Successful Aging™.  This program addresses the Cognitive, Occupational, Mental, Physical, Active, Social, and Spiritual needs of an individual’s well being.  A personal navigator (geriatric care manager) will be your residents’ guide as they create their personal roadmap to healthy aging.   Partners in Senior Care, Inc. will identify the top reasons for hospitalizations and leave of absences in your community. Such as:

  • COPD/CHF
  • Falls
  • Pneumonia
  • Urinary Tract Infection
Our Geriatric Care Manager will then help your residents create their Passport to Successful Aging.
